What is the Incentive Rewards Program?
The PCPS incentive program is designed to promote health awareness among employees. The program consists of completing an online health assessment (HealthQuotient) and one preventive care visit or screening. Employees enrolled in the School Board’s self-funded health plan will receive a $50 gift card upon completing these two (2) wellness activities.
Who is eligible to participate?
Active employees who are enrolled in the School Board’s self-funded health plan administered by Blue Cross Blue Shield of Florida (BCBS).
What do I need to do in order to receive the incentive?
- Complete the HealthQuotient
- Complete a preventive care visit or screening with your doctor, or at one of the two Employee Health & Wellness Center locations
Where can I go to complete the HealthQuotient?
Go to FL.ExlploreMyPlan.com/myBlueWellness
. You will be directed to log in using your username and password. Select “Healthy Living,” then choose “HealthQuotient” under “Access Your Health.”
Will my supervisor be able to see my responses to the HealthQuotient?
No. Neither your supervisor nor your co-workers will be able to view your individual responses. Only aggregate, de-identified results with be shared with the administrators of the School Board’s self-funded health plan. PCPS’s wellness program is administered according to federal rules permitting employer-sponsored wellness programs that seek to improve employee health or prevent disease, including the Americans with Disabilities Act of 1990 (ADA), the Genetic Information Nondiscrimination Act of 2008 (GINA), and the Health Insurance Portability and Accountability Act (HIPAA), as applicable. If you wish to receive a copy of the Notice Regarding Wellness Programs and/or the District’s Notice of Privacy Practices, please contact the Risk Management Department and they can provide you with these documents.
Where can I go to complete my preventive care visit?
You can complete your preventive care visit or screening through the Employee Health & Wellness Center by calling and scheduling an appointment: 863-419-3322
Alternatively, you may visit your regular primary care provider (PCP) within the community and schedule a preventive care visit or screening. You can search for in-network providers near you by visiting FL.ExlploreMyPlan.com/FindADoctor
What are some examples of preventive care activities that will count towards the incentive?
Examples of qualifying preventive care activities include Annual Physicals, , routine lab work/biometric screenings, and age-appropriate screenings that are classified as an A or B recommendation by the U.S. Preventive Services Task Force. This includes mammograms, cervical cancer screenings, prostate cancer screenings, etc. For a full list of preventive screenings covered by the plan, please visit FL.ExploreMyPlan.com/preventiveservices.
How much will it cost me to get a preventive screening/visit?
Employee Health & Wellness Centers: all services at the Health & Wellness Centers are free to employees.
In-network Provider: by being enrolled in the School Board’s self-funded health plan, you have access to free preventive care services when performed by an in-network doctor. During your visit, tell your doctor that the intent of the visit is preventive per the Affordable Care Act guidelines. Your doctor should not charge you preventive office visits or screenings. If you believe you were incorrectly charged for your preventive care visit, please contact your BCBS onsite representative or call BCBS Customer Service.
Where can I go for lab work/biometric screening?
Quest Diagnostics or the Employee Health & Wellness Centers. Please note: lab work must be prescribed by a doctor. You can visit an in-network doctor or a Health & Wellness Center provider to receive a prescription for your biometric screening.
Are my biometric screening results automatically uploaded into the wellness portal?
Yes. When registering on the myBlueWellness portal make sure you thoroughly read and review the HIPAA privacy statement. Your consent will allow for the automatic upload of biometric results.
If you do not consent, you will need to manually fill in your biometric screening results when prompted during your completion of the HealthQuotient.
Will I be able see if I completed the requirements for the incentive on the wellness portal?
Yes, you will be able to see when you have completed all your activities via the myBlueWellness Portal.
I completed my activities, when should I expect to receive my $50 gift card?
After you have completed your HealthQuotient and preventive care activity, you should expect to receive your $50 gift card via mail within 6-8 weeks. If more than 8 weeks have passed since the completion of your activities, please contact BCBS Customer Service.
